Request for Proposals

The Request for Proposals for 2012 - 2014 is complete.  We want to thank everyone who participated.  The following is the announcment information about the 2012 process.

 

NIAAA invites proposals from organizations to provide services for fiscal year 2012 (October 1, 2011 to September 30, 2012).  Fiscal year 2012 is the first year of the area plan which goes through 2014. The results of the Request for Proposal will determine the service provider organizations NIAAA intends to fund for services in 2012-2014.

 

NIAAA administers programs for older adults in the nine counties of northwestern Illinois — Boone, Carroll, DeKalb, Jo Daviess, Lee, Ogle, Stephenson, Whiteside and Winnebago counties.

 

NIAAA is funded through the Older Americans Act and Illinois General Revenue Funds (GRF) and is designated as an area agency on aging by the Illinois Department on Aging.  The following services are open for bid and the anticipated funding available in FY12:

 

- Community Based Services (Title III-B/General Revenue Funds) $521,459;

- Congregate Meals (Title III-C-1) $497,170;

- Home Delivered Meals (Title III-C-2 GRF) $1,311,822

- Health Promotion (Title III-D) $29,750;

- Medication Management (Title III-D) $11,287;

- Caregiver Services (Title III-E) $135,000; and

- Grandparent Information and Assistance (Title III-E); $20,000.

 

Organizations interested in applying for funding must submit a complete FY12-14 application to the NIAAA office by 4 p.m. on August 19, 2011.  The Application (in either Word or PDF format) along with RFP announcement, Instructions, and the Serive Provider Manual can be downloaded below.  Upon request, these documents will be emailed.  For other arrangements, please contact our office.
